Comparative in vitro activity of four respiratory fluoroquinolones (Resp-FQ) against clinical isoltes of strains of Streptococcus pneumoniae (SP).

Background: Resp-FQs are being recommended and widely used for the treatment of community-acquired pneumonia because of increasing multidrug resistance in SP. There is limited data comparing all of these agents "head to head".

Method: We performed broth microdilution testing using NCCLS recommendations and interpretative criteria on 2421 clinical isolates of SP obtained from ongoing cross-Canada surveillance between 1997 and 2000.

Results: Resistance rates were 6.1%, 2.1%, 1.8%, 1.0%, and 0.3% for ciprofloxacin (cip), levofloxacin (lev), gatifloxacin (gat), moxifloxacin (mox), and gemifloxacin (gem), respectively. We used a breakpoint of 31µg/ml to define gem resistance. The MICs of the 147 cip-resistant strains (MIC34µg/ml) are summarized below:

Conclusions: Greatest activity against both cip-sensitive (MIC£2µg/ml) and cip-resistant strains is as follows: gem>mox>gat>lev.
