Comparative In Vitro Activity of Beta-Lactams, Macrolides, Fluoroquinolones and Ketolides against Respiratory Pathogens in Quebec in 2002.

Background: Increasing resistance among respiratory pathogens is a growing challenge for clinicians. Continuous monitoring of resistance levels is necessary in order to optimise therapy.

Method: Throughout a surveillance network covering the whole province (17 centers), we have collected and tested all Streptococcus pneumoniae (Sp), Hemophilus influenzae (Hi) and 100 Moraxella catarrhalis isolated in these centers from 1 October 2001 to 31 March 2002. 407 Sp (295 non-invasive,112 invasive) were collected during this period. For Sp, 250 strains were isolated from men, 157 from women. 291 (71.5%) came from patients 16 and over.

Results: For Mc, the % of beta-lactamase producing strains was 79.6 %. All the other antibiotics were very active against Mc strains.

Conclusions: Resistance levels to all classes of antibiotics were stable compared to the previous year. There was a significant difference in terms of resistance to macrolides and beta-lactams between metropolitan areas and other areas in spite of similar antibiotics consumption. New ketolides (ABT-773 and telithromycin) showed great in vitro activity against Sp strains resistant to macrolides.
